  • Inquiries by phone: 9:00 - 17:30 on weekdays (available to receive general inquiries but not available to get inquiries on the dental departments during lunch time)
  • Office hour: 9:00 - 18:00 on weekdays (lunch time from 12:30 to 13:30)
  • Registration: Weekdays 8:30 - 11:00, and 13:30 - 16:00 (available during lunch time)
  • ※ Closed on weekends and holidays

How to register

You should visit the hospital first. (Reservation via internet or phone is not available)
  • Fill out the application form
    • Fill out the application form* and questionnaires at the application desk (where patients fill out the application form) in the lobby (on the first floor) and then submit it to the front desk (for registration and payment)
      ※ If you have any difficulties with the registration, our staff will help you.
      (*Application form : fill out patient name, resident registration number, address, telephone number and privacy agreement).
    • Patients can apply for General Practice (Special and inpatient treatments are not provided) provided by clinical professors (specialists) of dental hospital.
    • For new patients, in order to be assigned an appropriate department for treatment among various examinations of the dental hospital in accordance with the patients’ symptoms, a preliminary medical examination is conducted and then once a department for treatment is assigned, a staff in charge will inform the patient of the detailed procedure.
    • (If you are a patient who would receive a preliminary medical examination, please present it to the desk at the cooperative care center on the first floor and then wait there)
    • Registration time on the day of patient’s visit: Weekdays : 8:30 - 11:00, and 13:30 - 16:00 (available during lunch time, no treatment provided on weekends)
  • Payment
    • If you receive the paper stating payment information, please take a number, and go to the desk where the same number shows up on the electronic signboard, and then pay for your treatment.
    • When you pay for your treatment, the hospital does not receive the fee for the next medical appointment. So, please pay for your treatment on the day of your next appointment by visiting the front desk (for registration and payment) before directly moving to the department for treatment. For drug, please receive a prescription from the front desk (for registration and payment) and purchase the drug at pharmacies outside the hospital.
  • Booking the next appointment
    • The appointment for the next treatment is scheduled by the desk of department at which you received the treatment, before you return home.
    • If you want to know information such as the reservation date, time, the department and the name of your doctor, please check the appointment schedule listed on your medical expenses bill. On the day of the appointment, please visit the front desk (for registration and payment) first to pay for your treatment and to know the location of your department before moving to and registering at the department.
